Appeal Request Service
You may submit a written notice of appeal to ICD for review. ICD will review the request and you will be notified whether your appeal request is granted or denied.
If your appeal request is accepted by the ICD Appeals Committee, you will receive a letter outlining the next steps in the process. There is a service fee of $500 for the appeals process. This fee will be returned if your appeal is approved and the original decision overturned.
