ICD Physical Therapist Credentials Evaluation Application
Please remember to read all instructions very carefully, then fill in the forms neatly and completely. Once all forms are completed:
- Send the completed Application, items requested in the enclosure checklist and your payment to: International Consultants of Delaware, 3600 Market Street, Suite 450, Philadelphia, PA 19104-2651 USA.
- Forward all Request for Academic Records/Transcripts for Credentials Evaluation forms to the appropriate educational institution(s).
NOTE: Applicants must have received their ICD ID number before requesting documents from institutions or registration/license authorities. Documents without ID numbers will not be processed. - Forward all Request for Validation of Registration/License for Credentials Evaluation forms to the appropriate registration authority(ies).

Applicants who apply after March 2, 2009 will be given 12 months to meet the application requirements. If a report has not been issued within 12 months of the date the first evaluation checklist is mailed to the applicant, the application expires. Applicants may then apply to reprocess their expired application and pay the associated fee.
